HENRY’S HUMBLE HEART Adult (18+) Volunteer
Waiver/Release of Liability/Media Release

1. By signing this Waiver and Release of Liability (Agreement), I waive and release HENRY’S HUMBLE HEART, its
agents, servants, employees, insurers, successors and assigns from any and all claims, demands, causes of action,
damages or suits at law and equity of any kind, including but not limited to claims for personal injury, property
damage, medical expenses, loss of services, on account of or in any way related to or growing out of my presence
or involvement at the facility or away from the facility. This waiver and release is intended to and does release
HENRY’S HUMBLE HEART from any and all liability for damages or injuries on account of or in any way related to
or growing out of my negligence, the negligence of third parties and HENRY’S HUMBLE HEART’s negligence. This
is not intended to release HENRY’S HUMBLE HEART from any liability resulting from their intentional conduct. I
agree to indemnify and hold HENRY’S HUMBLE HEART and all its officers, employees, representatives, and
volunteers free and harmless from and against all claims, damages, losses and expenses, including attorney fees
that I may sustain while participating in the volunteer activity. I hereby release and discharge HENRY’S HUMBLE
HEART and all its officers, employees, representatives, and volunteers from all claims, demands, causes of action
of any nature or cause, for any such injury or damage incurred or suffered.

2. I understand that HENRY’S HUMBLE HEART is not responsible for any lost, stolen, or damaged valuables or
property.

3. I acknowledge that I have received and read a copy of the current rules and regulations governing my conduct at
and away from the facility. I agree that I will fully comply with all rules and regulations including the company
safety and fleet policies. I have read the Agreement and understand that by signing the Agreement I have
consented to be bound by its terms, including the waiver/release of any legal right I may have to sue HENRY’S
HUMBLE HEART for any costs they incur because a claim or legal action is brought in violation of this Agreement.
4. I acknowledge and agree that activities performed will be performed strictly on a voluntary basis, without any
pay, compensation, or benefits. I agree and understand that I must comply with the rules and regulations
established from time to time by HENRY’S HUMBLE HEART and that failure to do so may result in my immediate
removal as a volunteer.

video and photography taken of me for purposes of distribution on their website, social media channels, and print
materials.

Volunteer Signature
I have carefully read this agreement, waiver and release and fully understand its contents. I am aware that this is a
release of liability and a contract between HENRY’S HUMBLE HEART and myself, and I sign it of my own free will.
